-Complete this form for one or more (up to 4) addresses you wish a transcript to be sent.
-There is a prepaid charge of $10.00 for each transcript.
-ORMA reserves the right to withhold transcripts if the student’s financial obligations to the school are not paid or are overdue.
-Please allow up to 5 business days for processing transcript requests (plus shipping time).
